Former Foster Youth, We Got Your Back
When young people age out of foster care at age 18, they often struggle with common tasks like maintaining a job, applying to college and getting their first apartment. Wellpoint Care Network’s Youth Transitioning to Adulthood program aims to support former foster youth as they navigate life as an adult on their own.



Supporting Youth Leaving Care
Youth ages 18-21 who have exited, or will be exiting, out of foster care in Milwaukee County qualify for these services.
We we work with youth to achieve success in the Five Pillars of Stability, which include:
- Education: We help youth finish school, take the GED and can even get them a $5,000 scholarship for school.
- Employment: We help youth find a job that works with their skills and schedule, and make sure they maintain it.
- Housing: Our program has helped countless youth secure and furnish their first apartment.
- Health: We can answer your insurance questions and refer you to our outpatient mental health clinic.
- Caring connections: Case managers serve as mentors, teachers and advocates. Through compassionate mentoring and a focus on concrete skills, our case managers stand by youth every step of the way.
The YTA program is a partnership with the Wisconsin Department of Children and Families. Wellpoint Care has been proud to partner with the department in delivering independent living services since 2007.

GRiD MKE
When youth age out of foster care, it’s far too easy for them to fall off the grid and not have access to services to help them successfully enter into adulthood. GRiD MKE changes that. The app provides hundreds of resources in the areas of education, employment, health and housing as well as basic or emergency needs, like food and clothing pantries.
