Connecting Communities with Vital Resources
Research and experience have proven that when a family has all of its basic needs met, they are better able to thrive as a family, as well as individually. That’s why Wellpoint Care Network’s Community Connections team provides the community with resources that can lead to strong, stable lives.
Community Connections is a FREE, voluntary program that can connect you to the resources and support that families need the most. We focus on key areas of basic needs we call the Five Pillars of Stability. We’re happy to provide families with useful resources in any (or all) of the pillars they may need to thrive.
How the Five Pillars Can Help You and Your Family Thrive
The Five Pillars of Stability are Employment, Education, Housing, Health and Caring Connections. These pillars describe what it takes for an individual to truly thrive. That’s where we come in. Wellpoint Care‘s Community Connections team will partner with you one-on-one to help you achieve stability and reach your family’s fullest potential in areas like:

- Education: We can connect you to learning opportunities that build everyday skills. This includes help with GED or HSED programs, tutoring, parenting classes and college prep.
- Employment: We’ll support you in finding and keeping suitable employment that can provide financial stability. Get help with job training, resume building, money management and more.
- Housing: We’ll help you find safe, stable housing — and keep it. This could include affordable housing options, assistance with rent or energy bills, access to local shelters or financial advice.
- Health: Coaching on the importance of identifying providers in the community for health needs, as well as support in accessing mental health services.Your health matters — both body and mind. We’ll help you find access to clinics, dentists, mental health care, addiction recovery, food programs and other health services.
- Caring Connections: We help you build and keep strong, supportive relationships. This could mean joining a parenting or faith group, getting mental health support or taking part in community events.
